My in-app purchase didn't work on the iPhone app
I tried downloading a new dose from the iPhone app. It said that the purchase was completed successfully, and it deducted my account, but the option to start the dose isn't there. I can only buy it again. When I try that, I would expect that it would say that I've already purchased it, but it prompts me to load more money onto my account. I tried deleting the app and reinstalling, but it didn't help.

You need to re-download when you are logged into the same itunes account and password you used to originally purchase the dose. It will then prompt that you already purchased. Sounds like you are using a different itunes account. The files are large and take awhile to download. When download is complete, it will allow you to play it. Open and close the app, try rebooting the phone - there is nothing wrong with the app or the purchase process so it's something on your end.
